** This is the only working nuget package version of CouchDBProviders to date. **
- Fixed the nuget package where after adding a files element dll was not included.
- Changed the readme.txt example configurations to reflect better defaults.
- Changed log4net refernce to 1.2.11 explicitly 1.2.12 or 2.0.1 changes the signing key which is a breaking change
- Added a transform file to include a default log4net configuration section

If you downloaded a 4.0.1 or 4.0.2 version of this software it will not work properly, please update to 4.0.3
